Different states have different questions on their death certificates, some refusing still to use the U. S. Standard Death Certificate. This loses some names. Also, there are no penalties for misreporting or misclassifying a maternal death, so no wonder the count is less than the actual. Meanwhile, women are subjected to many unnecessary interventions, some of which kill. We need to study what other countries do—countries with lower death rates than ours.
